| Abstract | 「要旨」【目的】脆弱性骨盤骨折(fragility fractures of the pelvis;FFP)に手術加療を施行した症例の患者背景, 周術期合併症, 短期成績を検討すること. 【対象】2019年10月〜2022年10月までの間に手術加療され, 術後6か月以上フォロー可能であったFFP症例42例, 平均年齢83.5(65〜96)歳, 男性1例, 女性41例を対象としている. 検討項目としてRommens分類, 手術方法, 再手術率, インプラントのゆるみ, 周術期合併症, IVR施行率, 周術期輸血の適応率, 術前および最終フォロー時のParker mobility score(PMS), 術前, 術後3日目のnumerical rating scale(NRS)を調査した. 【結果】Rommens分類の内訳はtype IIa 2例, type IIb 13例, type IIIa 8例, type IIIc 3例, type IVb 13例, type IVc 3例であった. 手術は経皮的screw固定のみで固定し得た症例が35例(83.3%)であり, plate固定を併用した症例は7例であった. インプラントのゆるみはminorなものも含めると16例(38.1%)で認め, 1例でインプラント抜去を要した. 術中合併症としてscrewのminor perforationを6例で認めたが, 明らかな神経症状を伴う症例は認めなかった. 受傷時にIVRが3例で適応された(7.1%).入院時内科的合併症を有していた症例は30例であり(71.4%), 術後肺炎による死亡例を1例認めた. 受傷前のPMSは平均6.51点であり最終フォロー時のPMSは平均5.38点であった. 術前, 術後3日目のNRSは術前平均5.26点, 術後平均3.49点であり有意な改善を認めた(P<0.01). 【考察】FFPに手術加療を要した42例をretrospectiveに検討した. 80%以上の症例が経皮的screw固定手術で対応可能であったが, 受傷時の出血性ショックおよびIVRの適応も常に念頭に置く必要がある. インプラントのゆるみはminorなものも含めると38.1%に認めており, cement augmentationの使用など今後改善されるべき課題と考えられた. 術後3日目のNRSは術前と比較し有意な改善が確認されており手術加療が早期離床に寄与し得る可能性が示唆された. |
