Abstract | 〔論文要旨〕 国内の梅毒報告数は近年増加し続けていたが, 2019年は減少した. ただし, 性的接触による感染を異性間と同性間に分けた場合, その傾向は異なる. 異性間性的接触者の梅毒報告数は男女ともに増加から減少に転じた一方で, 男性同性間性的接触者(men who have sex with men, 以下MSM)の梅毒報告数は増加が継続している. 国内のHIV感染者および男性の梅毒症例の多くがMSMであることから, MSMにおいては特に, HIV感染症と梅毒の動向をともに注視していくことが重要である. また重複感染の場合, 梅毒がHIV感染者のパラメーター(HIV-RNA量やCD4陽性リンパ球数)に影響を及ぼすことや, 非典型的な梅毒の経過をとることなども報告されており, 臨床医においては, これらの臨床像を理解したうえで早期診断・治療に努める必要がある. 梅毒の発生動向に変化がみられつつあることから, 今後はより一層, 臨床医からの確実かつ正確な届出と, それらの人・時・場所の観点からの継続したモニタリングが重要である. |

Abstract | Although the number of reported cases of syphilis in Japan increased in recent years, there was a decrease in 2019. However, the trend differed between heterosexual and non-heterosexual populations; the number of reported syphilis cases among those with heterosexual contact began to decrease in both men and women, whereas that among men who have sex with men (MSM) continued to increase through 2019. As most persons living with HIV in Japan are MSM and a considerable number of reported male syphilis case-patients are MSM, it is important to pay particular attention to both trends in MSM. In the case of coinfection, syphilis may affect key parameters of HIV (e.g. plasma HIV-RNA and CD4-positive lymphocyte count) and lead to an atypical clinical course of syphilis. Therefore, it is imperative that clinicians understand these clinical features for early diagnosis and treatment. As the trends in syphilis continue to evolve, reliable and accurate notifications from clinicians will continue to be valuable, along with the careful monitoring of such data from the perspective of person, time, and place. |
