| Abstract | 〔論文要旨〕B型肝炎は小児期でも劇症肝炎, 慢性肝炎, 肝硬変, 肝細胞癌を起こす. 1985年からB型肝炎母子感染防止事業が始まり, 小児科領域で経験するB型肝炎は劇的に減少している. 現在までに開発されている抗ウイルス薬は, HBVの増殖を抑制するが, 完全に排除することはできない. したがって小児期の慢性肝炎の現実的な治療目標は, ALTの正常化, セロコンバージョン, ウイルス量の低下を達成して肝硬変, 肝細胞癌への進展を防ぐことである. 現在, 小児B型慢性肝炎の治療法としてはインターフェロン(interferon, 以下IFN), 核酸アナログ製剤(nucleoside analogues, 以下NA)がある. IFNはNAとくらべるとウイルス増殖抑制作用のほかに免疫賦活作用があるために, 長期的に効果的なことが多いので第一選択薬である. IFN治療は治療期間が24〜48週と短い, 耐性ウイルスを生じない, 催奇形性がない, などの特徴がある. NAは経口薬であり, IFN療法にくらべて通院回数が少ない, 副作用が少ない, などの利点があるが, 長期間服薬するのが原則である. 以上の要点は2018年公表の小児B型慢性肝炎に対する初回治療ガイドラインに記載されている. |

| Abstract | Hepatitis B virus (HBV) infection causes fulminant hepatitis, chronic hepatitis, liver cirrhosis, and hepatocellular carcinoma during childhood. Oral antiviral agents can suppress HBV replication, but cannot eradicate the virus. Thus, practical goals in treatment for chronic hepatitis B (CHB) consist of ALT normalization, HBeAg seroconversion, and suppression of HBV replication. Currently, treatment options for children with CHB comprise interferons and nucleoside analogues (NAs). IFN is the first choice because it has immune-promoting and antiviral effects, leading to long-term effects. In addition, IFN is advantageous in short-term regimens, does not induce resistant viruses, and has no teratogenic effects. In contrast, NAs require fewer clinical visits and produce fewer side effects, but should be taken long-term. The guidelines for the first-time treatment in children with CHB published in 2018 present essential parts of the content described above. |
