| Abstract | 「はじめに」慢性心不全治療の基本は, エビデンスに基づいたガイドライン推奨治療の遵守である. 既に, 左室駆出率の低下した心不全(HFrEF)に対する標準治療薬として, アンジオテンシン変換酵素(ACE)阻害薬あるいはアンジオテンシンII受容体拮抗薬(ARB), β遮断薬およびミネラルコルチコイド受容体拮抗薬(MRA)の有効性は確立している. いっぽうで, 左室駆出率の保持された心不全(HFpEF)に対して大規模臨床試験で有効性が確立した薬剤はないが, そのサブカテゴリ解析で有効な可能性が示唆される患者群が存在するのもまた事実である. 近年, アンジオテンシン受容体ネプリライシン阻害薬(ARNI)や過分極活性化環状ヌクレオチド依存性(HCN)チャネル遮断薬といった新たな治療薬が登場した. さらには, これまで糖尿病治療薬として用いられてきたナトリウム・グルコース共役輸送体(SGLT2)阻害薬が心不全治療薬としても承認され, 心不全の薬物治療も新時代に突入した. |
