| Abstract | 「要旨」: 夜間頻尿の原因は多岐に渡っており, 原因に応じた治療戦略の選択が重要となる. 前立腺肥大症は夜間頻尿の原因の一つだが, それに対する薬物治療のうち, α1ブロッカーの多くにおいて, 夜間頻尿に対する有効性を示す報告が多くあり, また夜間尿量を減少させる可能性も報告されている. 過活動膀胱も夜間頻尿の原因の一つである. その薬物治療として, 抗コリン薬, β3作動薬などが挙げられ, いずれにも夜間頻尿の改善効果が報告されているが, 抗コリン薬の種類によっては夜間多尿を改善させることがある. 睡眠障害も夜間頻尿の重要な原因の一つであり, その治療戦略は原因によって異なる. 基礎疾患が存在する場合はそちらの治療を優先し, 睡眠薬は患者の日常生活に重大な支障を来している場合に考慮される. 夜間多尿は夜間頻尿の主な原因である. 夜間多尿に対する新たな薬物療法として, デスモプレシンが保険収載され, 今後が期待される. |

| Abstract | [Abstract] Nocturia has many causes, and it is important to select the appropriate therapeutic strategy based on the particular cause of a patient's nocturia. Benign prostatic hyperplasia is one of the causes of nocturia. Beneficial effects of alpha-1 blockers (e.g., tamsulosin, naftopidil and silodosin) have been shown for nocturia, and several reports indicated the possibility that alpha-1 blockers might decrease the nocturnal urine volume. Overactive bladder is also a major cause of nocturia; anticholinergic medicines and beta-3 agonists have been shown to be effective for nocturia, and some types of anticholinergics were reported to exhibit a nocturnal urine-volume reduction effect. Sleep disorders are also important causes of nocturia. In many cases, sleep disorders have some underlying disease, and it is thus necessary to treat the underlying disease first. Hypnotics may be considered when a sleep disorder seriously disrupts a patient's daily life. Nocturnal polyuria is the major factor of nocturia. Low-dose desmopressin as a new medicine to treat nocturnal polyuria was approved in Japan in 2019. |
