Title 新人看護師のリアリティ・ショックが起こりやすい時期の離職予防支援に向けた考察

Abstract 「要旨」: 【目的】臨床1年目の看護師はリアリティ・ショック現象で気分が不安定になり, 落ち込む時期がある. 本研究では, 新人看護師のリアリティ・ショックが起こりやすい時期の離職予防支援を検討するために新人看護師の3カ月毎の感情の変化と看護労働環境との関連を明らかにすることを目的とする. 【方法】新人看護師を対象に, 臨床1年目の感情の変化と看護労働環境(Nursing Work Index-Revised: 6因子23項目)について質問紙調査をした. 分析は, 3カ月毎(1期; 4〜6月, 2期; 7〜9月, 3期; 10〜12月, 4期; 1〜3月)に感情の変化を3群(上昇群, 平行群, 下降群)に分類し, NWI-Rとの関連を検定した. 【結果】分析対象は24名(回収率43.9%). 1期では, 【看護管理者のマネジメント能力】の下位項目<看護師長による看護師のバックアップ>で, 感情の下降群に比べて平行群が有意に高かった(p=.038). 2期では, 【キャリア・アップ支援】の下位項目<キャリア・アップを図る機会>と, 【組織内における個人の位置づけ】の下位項目<院内や看護部門の委員会の委員となる機会>で, 下降群に比べて上昇群が有意に高かった(p=.015〜.028). 3期と4期は, 有意差はなかった. 【結論】新人看護師の気分が落ち込みやすい6月を含む1期では, 看護師長のバックアップを得にくいと捉えている場合, 感情が下降する傾向にあった. 2期では, キャリア・アップを図る機会や院内で役割を担う機会があると捉えている場合, 感情が上昇する傾向にあった. リアリティ・ショックが起こりやすい時期には看護師長のバックアップが得られていると感じられるような環境や, キャリア形成に力を入れたり自身も組織内の責任ある役割を担う機会があるのだと自覚する環境では, 新人看護師の感情が高まり, 離職予防に役立つことが示唆された.

Title Study on Support Measures for Preventing Turnover of Novice Nurses During the Period when the Reality Shock Phenomenon is More Likely to Occur

Abstract [Purpose]: Due to the reality shock, phenomenon nurses in their first year of clinical experience tend to become depressed with unstable feelings. This study aims to identify the relationship between the emotional changes of novice nurses during three-month intervals and their nursing work environment in order to examine support measures for preventing turnover of novice nurses during the period when the reality shock phenomenon is more likely to occur. [Methods]: A questionnaire survey was conducted with novice nurses in the first year of clinical experience to investigate their emotional changes and the nursing work environment, using the Nursing Work Index-Revised (NWI-R) scale (23 items in 6 factors). Dividing the emotional changes into rising, flat, and declining in four periods (1: April to June; 2: July to September; 3: October to December, and; 4: January to March), the relationships with the NWI-R were analyzed. [Results]: The analysis included 24 respondents (43.9%). In the 1st period, the flat group showed statistically significantly higher scores in <Support for nurses by the head nurses>, a sub-item of Nursing administrator's management skills' than the declining group (p=.038). In the 2nd period, the rising group showed statistically significantly higher scores than the declining group in <Opportunities for career development>, a sub-item of "Career up support," and in <Opportunities to be a member of hospital or nursing department committees>, a sub-item of "Individual standpoint in nursing organization" (p=.015-.028). [Conclusions]: Emotions tended to decline when these nurses felt difficult to obtain the head nurse support in the 1st period including June when novice nurses are prone to be depressed. In the 2nd period, emotions tended to rise when the nurses think that they have opportunities to develop their careers and take on roles in the hospitals. The findings suggest that an environment where novice nurses can perceive that they are supported by the head nurses during the period when the reality shock phenomenon is more likely to occur and where they can develop their careers, and become aware of the need to take on roles in the organization may improve the emotions of nurses and help prevent their turnover.
