| Abstract | 「抄録」慢性心不全患者のエンド・オブ・ライフに向けた意思決定支援を明らかするために熟練看護師9名に半構造化面接を行い, 質的に分析し, 意思決定支援は3つの段階で見出された. 基盤づくりの段階では【意思決定支援者としての準備を整える】【意思決定支援のための連携体制の基盤をつくる】【支援のための関係性の構築と意思決定力の査定を行う】【患者・家族の価値や生き方を理解する】, 支援実施の段階では【チームで協働するための情報共有と調整を行う】【適切なタイミングで患者の病状理解のための教育と調整を行う】【患者と家族が双方の思いや情報を共有できるように調整する】【患者と家族の最善の選択を支える】【患者と家族の意思決定内容を慎重に確認し, 支える】, 支援評価の段階では【次の意思決定支援につなげるために個々の事例の支援を評価する】のカテゴリーを抽出した. その実践のための教育や各施設に応じた体制整備の必要性が示唆された. |

| Abstract | [Abstract] The purpose of this study is to identified decision-making support for the end of life for patients with chronic heart failure. The semi-structured interviews with nine skilled nurses were conducted and the interview data were analyzed qualitatively. The result revealed three phases of decision-making support. The following categories were identified in different phases: in phase of foundation building [getting support providers ready for decision-making support] [creating a foundation of the collaborative system for decision-making support] [building relationships and assessing decision-making skills in preparation for support] and [understanding the values and ways of living of the patient and their family]; in phase of implementation [sharing information and coordinating for team collaboration], [providing education and coordination for patient's understanding of their condition at appropriate time], [coordinating with patient and their family for sharing thoughts and information], [supporting the patient and their family to make the best choice], and [carefully confirming and supporting the patient's and family's will regarding medical treatment and care options]; and in phase of evaluation [evaluating individual cases for the next opportunity of decision-making support]. The results highlighted the necessity of education and the development of a system for each facility to be put into practice. |
