Abstract	「要旨」日本褥瘡学会が2009年に「褥瘡予防・管理ガイドライン」を発表して以降, 手術中の褥瘡予防対策が広まり, 褥瘡発生率は減少していると予想されるが, わが国での現在の術中褥瘡発生率は明らかではない. 「褥瘡予防・管理ガイドライン(第4版)」で推奨されている予防対策を行った状況下で, 手術体位別に手術中の褥瘡の発生率と要因を調査した. 褥瘡の発生率は, すべての体位0.29%, 仰臥位0.18%, 砕石位0.06%, 坐位0.00%, 側臥位1.40%, 腹臥位1.16%であった. 褥瘡発生率が低いため多変量解析で同定はできなかったが, 褥瘡発生の要因として, 仰臥位では上腹部開腹手術で開創器具の使用による圧迫, 砕石位では坐位への体位変換による仙骨部のずれ, 側臥位ではパークベンチ体位による側胸部や大転子部に生じる圧迫や下肢骨折手術時の支持部への摩擦やずれ力, 腹臥位では支持部に生じる体圧やずれ力が推測された.

Abstract	[Abstract] Since the publication of the "Guidelines for the Prevention and Management of Pressure Ulcers" by the Japanese Society of Pressure Ulcers in 2009, measures to prevent intraoperative pressure ulcers have been implemented on a widespread basis, and a decrease in the incidence of intraoperative pressure ulcers was predicted. However, the current incidence of intraoperative pressure ulcers has not been clarified in Japan. We investigated the incidence and factors of intraoperative pressure ulcers in different surgical positions when the preventative measures recommended in the "Guidelines for the Prevention and Management of Pressure Ulcers (4th Edition)" were performed. The incidence rate of pressure ulcers was 0.29% overall, 0.18% in the supine position, 0.06% in the lithotomy position, 0.00% in the sitting position, 1.40% in the lateral position, and 1.16% in the prone position. Although the overall low incidence of pressure ulcers prevented multivariable analysis, we predicted the following risk factors for intraoperative pressure ulcers: the use of retractors in open upper abdominal surgery in the supine position, positional change to the sitting position in the lithotomy position, park-bench position and lower-limb fracture surgery in the lateral position, and the prone position.
