| Abstract | 「要旨」急性陰嚢症は突発する陰嚢疾患の総称である. 精巣捻転が緊急対応を要する病態であるが, 精巣垂捻転や精巣上体垂捻転の頻度が高い. その他に急性精巣上体炎, 特発性陰嚢浮腫, Henoch-Schonlein紫斑病などがある. これらを概説したのち, 精巣捻転に関して, その原因と疫学, 鞘膜内捻転と鞘膜外捻転, 診断のための外陰部所見のスコアー化やカラードプラ超音波検査など新しいコンセプト, 用手的整復など治療方針についての議論, 近年に開発された手術手技の評価, 捻転時間と捻転度数などの精巣救済に影響する因子の解析, 造精機能や妊孕性への影響などについて, クリニカルクエスチョンとアンサー, 筆者の経験に基づくコメントの形式で提示する. |

| Abstract | [Abstract] Acute scrotum is a common emergency presentation. Although the most critical diagnosis is testicular torsion, torsion of the appendix testis and appendix epididymis is common in children. Other possible diagnoses include acute epididymitis, idiopathic scrotal edema, and Henoch-Schonlein purpura. This study presents different topics on and related to testicular torsion. These topics include genetics and etiology, intravaginal and extravaginal torsion, novel concepts in diagnostic scoring and imaging, presence of intermittent torsion, and relationship with undescended testis and testicular tumor. Furthermore, this study also discusses management controversies, such as manual detorsion, novel surgical techniques, duration and degrees of torsion, factors affecting testis salvage, and impact on spermatogenesis and fertility. |
