Abstract 心筋疾患既往のない妊産婦が, 原因不明の左室収縮能低下をきたす心筋症を「周産期(産褥性)心筋症」と称する. 現時点では「妊産婦」以外に疾患特異項目はなく, 除外診断病名である. そのため, 多様な疾患背景を含む疾患群と考えられる. 妊産婦死亡に直結する疾患であり, 早期診断が重要である. 一方, 息切れや浮腫などの心不全症状は, 健常妊産婦が感じる症状と似ているうえ, 周産期心筋症の危険因子である妊娠高血圧症候群や多胎を合併した妊産婦では, それらの症状がさらに増大することから, 診断遅延や重症化の要因になっている. 近年, モデル動物による病因についての基礎研究成果や国際共同遺伝子研究成果が報告され, 病因・病態の究明が進んでいる. その中で, プロラクチンの病態関与可能性が指摘され, 重症例に対するドパミン作動薬による抗プロラクチン療法が試みられている. 今後, さらなる病因究明と疾患特異的治療の開発が望まれる.
