| Abstract | 「I. はじめに」近年の医学の進歩で, さまざまな免疫抑制薬・生物学的製薬・分子標的薬などによる抗炎症・免疫抑制治療が可能となった. 米国Mayo clinicの医師, Philip S. Henchにより関節リウマチ患者に対して初めてステロイドが投与されてから70年が経過しているが, 現在では自己免疫性疾患への使用にとどまらず, 新型コロナウイルス感染症 (COVID-19) をはじめとするさまざまな領域においてもステロイドは重要な治療薬として位置づけられている. 一方で, これだけ医学が進歩した現代でも, ステロイドの副作用との戦いは終わりを迎えることなく続いている. 本稿では, リウマチ・膠原病内科医である著者の立場から, ステロイドの副作用とその対処法の一般論を紹介するとともに, 急性呼吸促迫症候群 (acute respiratory distress syndrome : ARDS) などの急性期の病態へのステロイド使用と副作用管理についても考察を加えていく. |
