Abstract 「要旨」食物アレルギー(以下FA)の学童のセルフケアの実際と子どものセルフケア拡大に向けた養育者のかかわりを明らかにすることを目的にFAの学童と養育者14組に半構造化面接を行い質的帰納的に分析した. 誤食予防は低学年では大人に聞く, 高学年は自分で判断すると発達段階で行動が異なった. 食行動を伴う人とのかかわりの調整力は高学年の方が高かった. アナフィラキシー(以下An)への対応は, 低学年より高学年でアドレナリン自己注射薬の知識が多かったが, 緊急時に周囲に助けを求める行動は共通していた. 子どものセルフケア拡大に向けた養育者のかかわりは, 低学年より高学年の養育者の方が誤食予防や人とのかかわりを子ども自身に促す認識や行動が多かったが, Anへの対応は高学年でも養育者が行っていた. 一方で多くの養育者は, 思春期頃までにAnへの対応も含めた子どものセルフケアが自立することを望んでいた. FAの子どものセルフケア拡大に向けて, 成長発達や養育者のかかわりだけでは育ちにくいAnへの対応能力を育てるヘルスケア専門職の支援が必要である.

Title Self-care for school children with food allergy and parent's involvement in expanding children's self-care

Abstract [Summary] This study aimed to clarify the actual self-care situation of school students with food allergies (FAs) and parents' involvement in expanding children's self-care. Semi-structured interviews were conducted with 14 sets of school students (6 - 11 years) with FAs and their parents; data were analyzed using qualitative inductive analysis. All students consciously avoided the food items to which they were allergic due to parental involvement in their early years. However, a difference was seen in behavior during the development stage: students in lower grades asked parents what they can eat safely, while those in higher grades judged it for themselves. Knowledge about using an EpiPen(R) during anaphylaxis was higher in the higher grade than lower grades. However, students in higher grades realized that its use in an actual situation is difficult, and seeking help in an emergency was common among students in all grades. Regarding "avoiding allergic food" and "adjusting relationships", parents of higher-grade students were more likely to leave children to perform these tasks independently than those of lower-grade students. Contrarily, parents of higher-grade students wished that tasks of "responding to anaphylaxis" and "adjusting to a safe environment" be performed themselves, rather than by children. However, many parents wished that their children developed self-care by adolescence including responding to anaphylaxis. To expand self-care of students with FA, acquiring the ability to respond to anaphylaxis requires healthcare professionals' support. The current study found that acquiring this ability solely through the effects of the process of growth and parental involvement is difficult.
