| Abstract | 「要旨」夜尿症の3大病因は夜間多尿, 排尿筋過活動, 覚醒閾値の上昇である. これらは脳機能の問題, 発達の遅れが主要因であり, 子供達が成長発達してくれば自然治癒が望める. 主な夜尿症治療の戦略はこれらの主要病因を軸に立てられている. 一方, 夜尿症には, 小児泌尿器科疾患を病因する症例も一部存在する. 小児泌尿器科疾患に気がつかず, 漫然と夜尿治療を継続することは避けなければならない. 夜尿症診療では男児の外陰部の診察を必ず行うが, その形態などは時に小児泌尿器科専門医を紹介受診するか迷うことがある. 少しの知識と診療のコツが, 夜尿症診療における小児泌尿器科疾患へのアプローチを容易にする. この総説では, 夜尿症をきたす小児泌尿器科疾患の中から異所性尿管(尿管異所開口), 後部尿道弁, 過活動膀胱を, 夜尿症の原因疾患ではないが夜尿症の診療においてしばしば遭遇する小児泌尿器科疾患として, 包茎, 埋没陰茎を取り上げ概説する. 本稿が日常の夜尿症診療に少しでも役に立てば幸いである. |

| Abstract | [Abstract] The three major causes of nocturnal enuresis are nocturnal polyuria, detrusor overactivity, and elevated arousal threshold. These are mainly due to brain function problems and developmental delay. Treatment strategies for bedwetting are based on these major causes. In some cases, pediatric urological diseases cause nocturnal enuresis. A little knowledge and practice tips facilitate the approach to pediatric urological disorders in bedwetting practice. In this review, ectopic ureter, posterior urethral valve, and overactive bladder are overviewed as the pediatric urological diseases that cause nocturnal enuresis. Phimosis and concealed penis are also outlined although they are not the causative diseases of nocturnal enuresis. I hope that this article will be of some help to your daily bedwetting practice. |
