| Abstract | 「要旨」 食道癌の治療では, 外科手術, 放射線療法, そして化学療法を組み合わせた集学的治療がこれまで行われ, 治療成績は近年益々向上しつつある. 本稿では, 化学療法について, 現在の標準療法に関して概説する. 術前あるいは術後補助化学療法として, 術前シスプラチン+5-FU療法が現在の標準療法である. 切除不能・再発食道癌での化学療法として, 1次治療ではシスプラチン+5-FU療法が標準療法である. 2次療法ではこれまでタキサンが用いられてきたが, 2020年にニボルマブ, ペムブロリズマブなどの抗PD-1抗体が食道癌に適応追加となり標準療法の1つとなった. 切除可能食道癌における集学的治療, 切除不能・再発食道癌における化学療法について, 近年種々の臨床試験が実施されており, 従来標準療法として用いられてきた殺細胞性抗癌薬に加えて, 免疫チェックポイント阻害薬, さらにその他の分子標的薬との併用, あるいは新薬の導入が今後益々期待される. |
