Abstract | 「要約」Human parechovirus (HPeV) は主に小児の感染性胃腸炎や呼吸器疾患患者から検出されるウイルスである. その中でもHPeV-3は軽症例がほとんどな一方で, 新生児や早期乳児例においては, 敗血症様の症状, 髄膜炎, 脳炎などの重症感染症との関連が報告されており, 新生児や乳児期に治療に難渋する血球貪食性リンパ組織球症 (Hemophagocytic LymphoHistiocytosis : HLH) もしくはその類似症状の発症とHPeV-3とが関連するという報告が散見される. 今回, HPeV-3感染を契機に高サイトカイン血症および肝機能障害などのHLHまたはその類似症状をきたし, 治療方針に苦慮した乳児期早期発症した3症例を経験した. 3症例全てでHLHの診断基準は満たさなかったが, その病態が高サイトカイン血症によりHLHへの移行状態と進展していると推察しステロイドパルス療法や免疫抑制薬投与を行い, 軽快を得た. 乳児期早期にウイルス感染が疑われるが, 対症療法のみでは改善が乏しい症例において, HPeV-3感染症関連のHLH様症状を示す症例の存在を認識し原因ウイルス検索を行うと同時に早期に適切な検査, 診断および治療を行うことが重要である. |

Abstract | [Abstract] Human parechovirus (HPeV) is a virus mainly detected in children with infectious gastroenteritis and respiratory diseases. HPeV-3 has been reported to be associated with sepsis-like symptoms and severe infections such as meningitis and encephalitis m newborns and early infants. There are some reports that HPeV-3 is associated with hemophagocytic lympho histiocytosis (HLH) or similar symptoms that are difficult to treat. We encountered 3 cases of early onset in infancy with HLH or similar symptoms such as hypercytokine storm and hepatic dysfunction triggered by HPeV-3 infection. Although the diagnostic criteria for HLH were not met in all 3 cases, it was considered that the pathological condition had progressed to a transitional state of HLH due to hypercytokinemia, and appropriate treatment was performed to improve the condition. In cases of suspected early infancy viral infection, it is important to recognize the existence of cases showing HLH-like symptoms related to HPeV-3 infection, search for the causative virus, and perform appropriate tests, diagnosis, and treatment at an early stage. |
