| Abstract | 「概要」目的:在宅看護の普及に伴い, 急性期病院から直接退院する患者が増えている. 本研究は, 急性期病院で勤務する看護師の看護実践能力と, 患者の退院後の生活を見据えた行動との関連を明らかにすることを目的とした. 方法:A県内の急性期病院3施設に勤務する看護師1,658名に, 自記式質問紙調査を行った. 患者の退院後の生活を見据えた看護師の行動は「在宅看護の質自己評価尺度」を, 看護師の看護実践能力は「看護実践の卓越性自己評価尺度-病棟看護師用-」を用いて調査した. 【患者の退院後の生活を見据えた行動】と【急性期病院で勤務する看護師の看護実践能力】の関連を調査するために, 在宅看護の質自己評価尺度の得点をもとに2群に分け, ロジスティック回帰分析を行った. 結果:質問紙は1,055部回収でき, 有効回答は889部(有効回答率53.6%)であった. 在宅看護の質自己評価尺度の平均得点は103.0 ± 22.1(標準偏差), 看護実践の卓越性自己評価尺度の平均得点は120.8 ± 18.5であった. 患者の退院後の生活を見据えた行動に関連する看護実践の卓越性自己評価尺度の下位尺度は, 【現状に潜む問題の明確化と解決に向けた創造性の発揮】【患者の人格尊重と尊厳の遵守】【医療チームの一員としての複数役割発見と同時進行】の3下位尺度であった. 結論:急性期病院に勤務する看護師の患者の退院後の生活を見据えた行動を強化するために, 3要因を高める具体的な教育や支援が必要であることが示唆された. |

| Abstract | [Abstract][Objectives] With the spread of home health care nursing, more patients have been discharged directly from their acute-care hospitals to their homes. This study was aimed to clarify the relationship between nursing competence and practice anticipating the life at home of patients after discharge in nurses working in acute-care hospitals. [Methods] A self-administered questionnaire survey was conducted among 1,658 nurses working in three acute-care hospitals in A prefecture, Japan. The "Scale on Home Health Care Nursing" was used to evaluate the practice anticipating the life at home of patients after discharge, and the "Nursing Excellence Scale in Clinical Practice" was to evaluate the nursing competence. To examine the subscales of the Nursing Excellence Scale in Clinical Practice related to the score of Scale on Home Health Care Nursing, the scores of the Scale on Home Health Care Nursing were divided into two groups, and a logistic regression analysis was performed. [Results] A total of 1,055 nurses responded, and the valid responses were obtained from 889 nurses (53.6%). The mean score of Scale on Home Health Care Nursing was 103.0+-22.1 (Standard deviation). That of Nursing Excellence Scale in Clinical Practice was 120.8+-18.5. Three factors were related to the score of Scale on Home Health Care Nursing. They were "Clarification of problems underlying in existing conditions, and creativity toward solution," "Respect for patient's personality and human dignity," and "Discovery and simultaneous process of multitask as a member of a health care team." [Conclusion] It is suggested that continuous education and support to enhance these three factors identified in this study are necessary to strengthen the nursing practice anticipating the life at home of patients after discharge in nurses working in acute care hospitals. |
