| Abstract | 「概要」 本研究の目的は病院における認知症高齢者の転倒予防のための系統的観察から対応, 評価に至る看護ケア項目を特定し, 臨床における転倒予防ケアの向上につながる示唆を得ることである. 研究デザインはデルファイ法を用いた量的記述的研究とした. 全国の老人看護専門看護師及び認知症看護認定看護師960名中281名を対象に, 郵送にて質問紙調査を2回行った. 各項目のコンセンサス基準は, 先行研究を参考に実施度の平均値を4.0以上, 重要だと判断した者の割合を80%以上と設定した. 129項目中121項目の同意となった. 認知症高齢者に対する転倒予防ケア項目としては, 「生活状況を査定し整える」「移動によるリスクを査定し整える」「認知機能の程度を査定しケアする」「原疾患やフレイルを査定し緩和を図る」「患者の視点で入院環境を査定し整える」「患者の情報を共有し一丸となってケアする」「患者の尊厳を擁護する」に大別された. センサーマットや身体拘束に関する項目などが適合除外となった. 本研究によって抽出された認知症高齢者の転倒予防ケア項目は, 認知症高齢者の認知機能や移動能力の査定だけにとどまらず, 認知症高齢者の意思や生活状況にも焦点を当てることで, 認知症高齢者の尊厳を維持しながら安心して入院加療するための看護実践につなげることができると考える. |

| Abstract | [Abstract] Purpose: This study aimed to identify nursing care items for systematic observation, nursing intervention and evaluation of fall prevention in elderly people with dementia in hospitals, and to obtain suggestions to improve fall prevention care in clinical practice. Method: The study design was a quantitative and descriptive study using the Delphi method. A newly developed self-administered questionnaire was distributed by mail to certified nurses in dementia nursing and certified nurse specialists in gerontological nursing, with whom an expert panel was organized. Established consensus was defined as 80%- or 4.0-point agreement on results of the second survey. Result: A total of 121 out of 129 items were agreed upon. Dementia prevention care items included "living conditions", "risks due to movement", "the degree of cognitive function", "diseases and frailty", "the hospitalization environment", "teamwork", and "protect the dignity of the patient". Items related to sensor mats and physical restraints were excluded from conformity. The fall prevention care items for the elderly with dementia extracted by this study focus not only on the assessment of cognitive function and mobility of the elderly with dementia, but also on the intentions and living conditions of the elderly with dementia.
