Abstract 「要旨」: 免疫抑制薬の進歩により拒絶反応は減少した一方で, 免疫力低下による感染症は今もなお大きな問題である. 尿路感染症と腎盂腎炎は, 腎移植レシピエントにおける細菌感染の最も一般的な形態であるが, カルシニューリン阻害薬の登場以降は, ウイルス感染が重大な問題になってきた. 既感染ドナーから持ち込まれたウイルスや, レシピエント内に潜伏していたウイルスの再活性化以外に, 新規感染による発症もある. BKウイルスは, 尿細管間質性腎炎(BKウイルス腎症)による移植腎機能喪失を引き起こすこともある. そのほか, 出血性膀胱炎や肉芽腫性間質炎を起こすアデノウイルス, 移植後リンパ増殖性疾患を発病させるEpstein-Barrウイルスがある. さらに, 免疫力低下による日和見感染症として, 間質性腎炎を起こすサイトメガロウイルスや真菌による感染症も知られている. 今回は, 腎移植でみられる感染症の腎病理像を概説する.

Abstract Advances in immunosuppressive drugs have minimized rejection and improved renal transplant outcomes. Alternatively, infections resulting from compromised immunity remain a major issue. Urinary tract infections and pyelonephritis are the most prevalent forms of bacterial infection in renal transplant recipients ; however, viral infections were the main problem after calcineurin inhibitors became the mainstream of immunosuppression; it is caused by the reactivation of viruses induced from previously infected donor organs or by latent viruses in the recipient, or de novo infection. BK virus can lead to graft loss due to BK virus nephropathy. Adenovirus causes hemorrhagic cystitis and granulomatous tubulointerstitial nephritis, and the Epstein-Barr virus causes post-transplant lymphoproliferative disease. Furthermore, cytomegalovirus causes interstitial nephritis, and fungal infections are also known to cause renal injury due to opportunistic infection. We reviewed the pathology of infectious diseases encountered in renal allografts in this article.
