Title 精神科長期入院患者へのリカバリー志向の看護実践に必要な能力 - 当事者の視座に基づく検討 -

Abstract 「要旨」本研究では, 地域で生活する精神障害のある当事者が看護師に求める能力に関する考えを基盤として, 精神科長期入院患者へのリカバリー志向の看護実践に必要な行動を整理し, その行動に関連する当事者の経験や考えを明らかにする. これらから, 当事者の視座を踏まえた精神科長期入院患者に対するリカバリー志向の看護実践に必要な能力を検討することを目的とする. 調査1では, 当事者13名にリカバリー志向の支援において看護師に求める能力に関連する考えについて半構造化個別面接を行った. この結果と先行研究の当事者にとってのリカバリーとその特性 (葛谷, 2025) を踏まえ, 精神科長期入院患者に対するリカバリー志向の看護に [必要な行動] を検討した. 調査2では, [必要な行動] に関連する経験や考えを収集するために当事者15名を対象にグループインタビューを3回実施した. 調査1では〔支援する-支援される関係や上下関係, 対病気の人という関係ではなく, 対等な立場で接してほしい〕など14の考えが示され, [必要な行動] として [1. 入院している人と対等な関係性を築くことができる] など17項目を挙げた. 調査2では【行動制限時の人権への配慮や看護師の対応が不十分でトラウマとなっている】など25の経験や考えを明らかにした. 精神科長期入院患者に対するリカバリー志向の看護実践には, 入院している人が自分の人生について話をしたいと思えるような関係性を構築する能力, 非自発的入院や強制的な治療の経験が権利侵害やトラウマにつながることを理解し倫理的に考え行動する能力, ストレングスの視点をもち, 入院している人を「地域生活を送る権利をもつ生活者」として理解し, 人生を尊重した支援を行う能力, 地域生活への移行を見据えて心身両面のセルフケアを高めるために支援する能力, 看護師にある前提や価値観を問い直し看護を変えていくために内省する能力の5つの能力が必要である.

Title Practical Nursing Competence Required for Recovery-Oriented Practice with Long-Term Psychiatric Inpatients : A Study Based on the Perspective of Service Users

Abstract [Abstract] This study aimed to clarify the actions required for recovery-oriented nursing practice for long-term psychiatric inpatients, based on the perspectives of individuals with mental disorders living in the community. Furthermore, it sought to explore participants' experiences and views related to these actions and examine the competencies necessary for recovery-oriented nursing practice from their perspective. In Survey 1, semi-structured individual interviews were conducted with 13 participants to collect their perspectives on competencies expected of nurses in recovery-oriented support. Based on these findings and previous research (Kuzuya, 2025) on the nature and characteristics of recovery from participants' viewpoints, 17 specific actions necessary for recovery-oriented nursing practice were identified, such as the ability to build equal relationships with inpatients. Survey 1 revealed 14 key perspectives, highlighting the desire of inpatients to be treated as equals, not in hierarchical or disease-centered relationships. In Survey 2, three group interviews were conducted with 15 participants to gather experiences and views related to these identified actions. Survey 2 revealed 25 experiences and perspectives, such as concerns about insufficient consideration for human rights during behavioral restrictions and inadequate nursing responses that caused trauma. To implement recovery-oriented nursing for long-term psychiatric inpatients, five key competencies were identified : (1) the ability to build relationships that encourage inpatients to talk about their lives ; (2) the ability to ethically understand and address rights violations and trauma arising from involuntary hospitalization and forced treatment ; (3) the ability to adopt a strengths-based perspective, understanding inpatients as individuals with the right to community living, providing support for respectful living ; (4) the ability to support the enhancement of physical and mental self-care to prepare for community living ; and (5) the ability to reflect on own assumptions and values to transform nursing practice.
