| Abstract | 「要旨」【目的】認知症の診断後支援の現状と課題, 特に軽度認知症と診断された高齢者やその家族の思いや体験への対応を含めた診断後支援に関する国内外の文献レビューを行い, 現在までの当該支援の報告を概観するとともに, 軽度認知症の診断後に求められている支援の抽出と今後の検討課題を明らかにすることを目的とする. 【方法】学術情報検索は, 医中誌Web, CiNii, PubMed, CINAHL, ハンドサーチによる検索を行った. 診断後支援は「診断後支援」AND「認知症」, 思いや経験については「軽度認知症」OR「早期認知症」OR「認知症初期」に「思い」OR「語り」OR「経験」を加えて検索を行い, 最終的に46文献を分析対象とした. 【結果】軽度認知症高齢者の診断後支援としては, 【支援者に求められる基本姿勢】, 【その人なりの病気の受容と今後の生活への支援】, 【地域・社会とのつながりに対する支援】の3つのカテゴリに分類された. 【結論】軽度認知症と診断された高齢者は, 一定の認知機能が保たれているがゆえに, 自身の症状に対する苦悩を抱えているが, 今後の生活に自分になりの考えを持っている. 支援者は"認知症"に焦点化するのではなく, 1人の人として尊重し, 家族を含めて対話を持ち, 支援内容を一緒に考えることが求められている. そのうえで, 本人なりに認知症とともに生きていく方法を身に着け, 住み慣れた地域で社会とのつながりを持ち続けて生活できるような支援の必要性が示された. 今後は諸外国の支援方法とその成果を参考にしつつ, 日本における軽度認知症高齢者の診断後支援として具体的な指針を作成していくことが必要と考える. |

| Abstract | [Abstract] [Purpose] This review of domestic and international literature aimed to identify the current and emerging postdiagnostic needs, including the need for support for expressing emotions and experiences, in older individuals with early-stage dementia and their families. [Methods] Domestic and international databases, including Ichushi-Web, CiNii, Pubmed, and CINAHL(Cumulative Index to Nursing & Allied Health), were used to identify relevant studies using the following search terms: "postdiagnosis support" AND "dementia", and "early-stage dementia" AND "perspective" OR "narrated" OR "experience." A total of 46 articles were included in the final analysis. [Results] The following categories related to the post-diagnostic support for older individuals with early-stage dementia were identified: [basic attitude required of supporters], [support for acceptance of the illness in their own way and future life], and [support for community and social connections]. [Discussion] Older individuals with early-stage dementia can maintain a certain level of cognitive function, and therefore, experience distress regarding their diagnosis. Moreover, these individuals also maintain their own ideas on how they want to live their lives in the future. Rather than focusing on "dementia, " supporters are expected to respect the person as an individual, hold a dialogue with them, and consider the content of support together. Therefore, the need for support persists to help affected individuals in learning how to live with dementia in their own way and maintain social connections in the community. Future efforts should aim to create specific guidelines for the post-diagnostic support of older individuals with early-stage dementia in Japan, considering the support methods and results obtained in studies from other countries. |
