Abstract 「抄録:」【目的】重症ピーナッツアレルギーに対する少量の食物経口負荷試験(OFC)を検討した報告は少なく, 至適負荷量を明らかにする. 【方法】2017年1月から2021年12月に, 昭和大学病院小児科で総負荷量0.1gまたは0.3gのピーナッツOFCを初めて行い, ピーナッツ即時歴あり, またはAra h 2特異的IgE抗体価が4.0UA/mL以上を対象とした. OFCは40分間隔2分割法(1/3-2/3)で行った. 【結果】対象は51例, 0.1g群33例, 0.3群18例であった. 群間で背景因子, 特異的IgE抗体価に有意差はなかった. OFC陽性率は0.1g群33.3%, 0.3g群72.2%で有意に0.3g群が高かった. 閾値量別の症状グレード比較では, 閾値0.03g(0.1gの1/3量)は, 閾値0.1g(0.1gの3/3量と0.3gの1/3量)と0.3g(0.3gの3/3量)より症状グレードが低い傾向があった.【結論】重症ピーナッツアレルギーのOFC総負荷量は0.1g以下が望ましい.

Title Appropriate dose for oral food challenge for severe peanut allergy

Abstract [Summary: Objective: Few studies have been conducted on low dose oral food challenge (OFC) for severe peanut allergy in Japan. We aimed to determine an appropriate dose for the OFC for severe peanut allergy. Methods: This retrospective study included patients who had allergic reactions to peanuts or had serum Ara h2-specific IgE levels of 4.0 UA/mL or higher and received a small dose (0.1 g or 0.3 g) of peanuts for OFCs at Showa University Hospital between January 2015 and December 2021. The OFCs were performed using an escalating 2-dose method involving an interval of 40 min. Results: In total 51 cases were included, of which 33 were from the 0.1 g group and 18 were from the 0.3 g group. No significant differences were noted in background factors and allergen-specific IgE levels in both groups. The OFC-positive rate was significantly higher in the 0.3 g group (72.2%) than in the 0.1 g group (33.3%). A comparison of symptom grades by positive threshold dose showed that patients with a positive threshold of 0.03 g (1/3 amount of 0.1 g) had lower symptom grades than those with a threshold of 0.1 g (3/3 amount of 0.1 g and 1/3 amount of 0.3 g) or a threshold of 0.3 g (3/3 amount of 0.3 g). Conclusion: The challenging dose for OFC for severe peanut allergies should be set to 0.1g or less.
