Abstract | 「要旨」 : 事例は認知症を始めとした重複疾患があり, BPSDにより, 目標共有に難渋した. 今回, 重複疾患事例に対してADOCを用いて本人の意思に沿った作業を抽出し, 音楽療法, 運動療法, ADL練習と段階的かつ複合的な介入を行った. 精神症状の安定化を図るために, 趣味である音楽鑑賞を最初に導入した. その結果, BPSDが軽減され, 積極的な運動療法の実施に繋がり, ADLは改善した. BPSDのある認知症者に対してADOCを用いることで, 本人の意思に沿った作業の選択ができ, 作業療法に取り入れることで早期に精神症状の安定化が図れたと考える. また, 対象者の精神面と身体機能面に配慮し, 段階的かつ複合的な介入を導入することが重要であると考えられた. |

Abstract | [Abstract] The patient had dementia with overlapping diseases and behavioral and psychological symptoms of dementia (BPSD), making it difficult to share her goals. In this study, occupations that respect the patient's will were extracted using the Aid for Decision-making in Occupation Choice (ADOC) for patients with multiple diseases presenting with dementia. A complex intervention based on the extracted occupations was implemented in stages using music therapy, exercise training, and activities of daily living (ADL) practice. Music therapy was introduced first to stabilize psychiatric symptoms. As a result, BPSD were reduced, leading to active exercise training. The use of ADOC in dementia patients with BPSD facilitates the choice of occupations that can be made according to their own will, and psychiatric symptoms may have been stabilized early by incorporating these tasks into the occupational therapy program. Complex interventions should be introduced according to the mental and physical functional aspects of the patient. |
