Title HPVワクチンキャッチアップ接種事業対象女性の接種行動を促進する因子

Abstract 抄録: 本研究の目的は, 2022年4月に開始されたヒトパピローマウイルス(HPV)ワクチンキャッチアップ接種事業の効果を把握し, 接種行動を促進する因子を探索することである. キャッチアップ接種事業の対象で定期接種の積極的勧奨を受けられなかった石川県内の若年成人女性(19〜22歳)を調査対象者とし, 子宮頸がんやHPVワクチン, 子宮頸がん検診の知識や態度といった健康信念に関する因子のWeb調査を2022年8〜9月に行った. 接種者を初回接種時期で事業開始前, 事業開始後に層化し, それぞれ未接種者と比して関連する因子について二項ロジスティック回帰分析を行った. 調査対象者287人のうち, 未接種者は233人(81.2%), 事業開始前の初回接種者は20人(7.0%), 事業開始後の初回接種者は34人(11.8%)であった. 事業開始後の接種行動を促進する因子は, 子宮頸がんやHPVワクチンに関する知識が高いこと, 接種後の検診受診の必要性の知識を有すること, 接種が面倒と思わないことであった. 事業開始前の接種行動を促進する因子であった高い年齢層, 副反応が心配と思わないことは関連がみられなかった. 事業開始後の初回接種者の約7割が母親の勧めで接種し, 母親や自治体から事業開始の情報を得て, 接種が子宮頸がん予防に有効と自ら認識した上で接種していた. 以上より, キャッチアップ接種事業の効果的な展開に向けて, 未接種者本人や母親への啓発, 行政からの情報提供体制, 医療機関での相談体制についての示唆が得られた.

Title Factors Facilitating Vaccination Behavior among Women Eligible for HPV Catch-Up Vaccination Project

Abstract Abstract: This study aimed to determine the effectiveness of the Japanese human papillomavirus (HPV) Catch-up Vaccination Project started in April 2022 and explore the factors facilitating vaccination behavior after the start of the Catch-up Vaccination Project. The study participants were young women (aged 19-22) who were eligible for the Catch-up Vaccination Project and had not received active recommendations for routine HPV vaccination. A web-based survey was conducted from August to September 2022 in Ishikawa Prefecture, Japan, to analyze health-belief-related factors such as knowledge of and attitudes toward cervical cancer, the HPV vaccine, and cervical cancer screening. Those vaccinated, were categorized into two groups: One group received vaccination for the first time before the project started ("vaccinees before the project started"), and the other group was vaccinated after the project started ("vaccinees after the project started"). Binomial logistic regression was conducted to determine the effective factors of "vaccinees before the project started" and "vaccinees after the project started" with respect to the unvaccinated participants. Of the 287 participants, 233 (81.2%) were unvaccinated and 54 (18.8%) were vaccinated; of the latter, 20 (7.0%) and 34 (11.8%) were "vaccinees before the project started" and "vaccinees after the project started", respectively. The factors facilitating vaccination behavior after the start of the project included higher knowledge about cervical cancer, the HPV vaccine, and knowing the need to undergo cervical cancer screening even after vaccination, and the low perceived hassle of vaccination. The factors of higher age-range and low perception of adverse reactions to vaccination were significantly facilitative of vaccination behavior before the start of the project but not significantly so afterwards. Approximately 70% of "vaccinees after the project started" received vaccination based on their mothers' recommendations. They obtained information regarding this project from their mothers or their municipalities' notification and they decided upon vaccination because of their perception that it effectively prevents cervical cancer. The above findings suggest that the following points should be considered for effectively developing the Catch-up Vaccination Project: the education of health literacy to unvaccinated young women and their mothers, and the information provision system from the government and consultation system in medical institutions.
