Abstract	「要旨」 脳卒中の発症により要介護状態となった患者の在宅復帰には, 本人の希望だけでなく家族が在宅介護をする意思を持つことが必要である. 本研究の目的は, 脳卒中患者の家族が在宅介護を決意するまでの意思決定過程を明らかにすることである. 発症後3年未満の脳卒中患者を在宅介護している家族11名を対象に半構成的面接を行い, 修正版グラウンデッド・セオリー・アプローチを用いて分析した. その結果, 脳卒中患者の家族が在宅介護を決定するまでの意思決定過程は【先のことまで考えられない】【回復を期待し, 転院を決める】【患者のこれから先の人生を支える決意をする】【理想と現実の間で揺れ動く】【患者のことを考えて退院を決意する】【退院に向けて在宅介護の意思を固める】【迷いを断ち切る】の7つのカテゴリから構成された. 医療者は脳卒中患者家族の揺れ動く心情や複雑に変化する意思決定の過程を理解し, 家族とともにこれからの生活を考える姿勢をもって支援していくことが必要である.

Abstract	For a stroke patient that requires long-term care to return home, it is necessary that their families have a strong intention to take home care as well as respecting the patients' request. The purpose of this study is to clarify the process of decision making until family members of stroke patients decide home care. A semi-structured interview was conducted on 11 home care families with stroke patients less than 3 years after onset. The collected data was then analyzed using a modified grounded-theory approach. As a result, the process of decision making until family members of stroke patients decide home care is structured from 7 categories as the following: [cannot think ahead] [expect to recover and decide to transfer] [determined to support the affected member's future] [emotionally alternating between hope and reality] [discharge from hospital for the good of the affected member] [confirm intentions of home care to discharge] [cutting off hesitation]. It is necessary for healthcare professionals to understand the shaking feelings of family members of stroke and the process of decision making that changes in a complex way and support them with the attitude of considering future living with their families.
