Abstract 「要旨」 経鼻弱毒生インフルエンザワクチン(LAIV)は2023年3月, 2歳以上19歳未満を対象に日本でも認可され, 2024年秋から本格的に導入されることになった. 2016-17年に国内第3相試験が実施され, 全インフルエンザに対する発病防止効果は28.8% (95%CI: 12.5-42.0)で, A(H3N2)に対する効果は28.8% (CI: 9.0-43.1)であった. またB型に対する効果は, 28.8% (CI: -42.2-64.3)で統計的に有意ではなかった. 年齢群別では, LAIVの主要な接種対象となる2-6歳群で有効ではなかった. LAIV接種に関連した重篤な有害事象は認められなかった. 米国ではLAIVがA (H1N1) pdm09に対して無効なことが報告され, 一時, 接種は中止されたことがある. 日本でも効果の検証が必須である. なお米国では, LAIVは自己投与または介護者による投与の申請中である.
