| Abstract | 「要旨」「目的」 : 胃瘻を造設していない在宅要介護高齢者の「食べること」について看取り時期の主家族介護者の認識の構造を明らかにする. 「方法」 : 訪問看護を利用し胃瘻を造設していない在宅要介護高齢者を看取った主家族介護者9人を対象とし, 「食べること」への認識について半構造化面接を実施, KJ法を用いて分析した. 「結果」 : 要介護高齢者の「食べること」への看取り時期の主家族介護者の認識の構造は3つに統合され, それらは【食べることは生・活そのものだ】を【食を含めた介護を全うすることが私の務めだ】が支えるが, やがて主家族介護者の意に反して【食べられなくなることに抗えなかった】の方向に進む関係であった. 「結論」 : 主家族介護者は, 在宅要介護高齢者の「食べること」を生存や活き活きすることと捉え, 食を含めた介護の責任を負いながらも, 食べられなくなることをどうすることも出来ないと認識していた. 主家族介護者の認識の構造を踏まえた看護の必要性が示唆された. |

| Abstract | [Abstract] [Purpose :] This study aimed to clarify the structure of the perception of the family members who were primary care providers assisting non-gastrotomy older adults requiring home nursing care in eating. [Method :] This study targeted participants included nine family members who were primary care providers assisting non-gastrotomy older adults requiring home nursing care with the help of home-visit nursing. A semi-structured interview was conducted on the perception of the participants toward "eating." Their responses were analyzed using the KJ method. [Result :] The structure of the perception of family members who were primary care providers of older adults requiring nursing care for "eating" was integrated into three. Family members support [eating as a means of sustaining life and an activity] by [providing complete nursing care that includes food]. However, the relationship eventually shifts toward [being unable to stop the patient from losing the ability to eat], which contradicts the intention of the primary care provider. [Conclusion :] Family members who were primary nursing care providers associate the "eating" of older adults requiring home nursing care with survival and vitality. They recognize that they were unable to do anything when the patient loses the ability to eat while taking responsibility for nursing care, including food. It was suggested that nursing care could be based on the perception structure of the family members who were primary care providers. |
