Abstract | 「要旨」目的: 本研究の目的は, 慢性閉塞性肺疾患(COPD)患者の急性増悪後を生きる体験を明らかにすることである. 方法: 急性増悪後, 自宅に退院したCOPD患者5名に非構成面接を実施し現象学的な手法を参考に質的記述的に分析を行った. 結果: COPD患者の急性増悪後を生きる体験として【息苦しさが身体の一部になる】【身体の声を聴き身をゆだねる】【過去への後悔から生活を立て直す】【今の生活を維持するための継続的な努力】【あたりまえの生活を送れるありがたさ】【命の限りを意識しても生きる希望を持つ】【持てる能力を発揮し病とともに生きる】の7つテーマが抽出された. 結論: COPD患者は呼吸機能の低下により死を覚悟していたが, 過去の生活を振り返り自身の生き方を変容させてでも生き抜こうと, 主体性をもって生活していることが明らかになった. 患者の全人的な理解と支援には, 身体症状だけではなく過去の生活体験にも目を向ける必要があると示唆された. |

Abstract | [Abstract] Aim: The aim of this research is to identify how patients with chronic obstructive pulmonary disease (COPD) experience life after acute exacerbation. Methods: We conducted unstructured interviews with five COPD patients discharged from the hospital after acute exacerbation, and analyzed their answers qualitatively and descriptively applying a phenomenological approach. Results: Seven themes were identified in the way the COPD patients experienced life after acute exacerbation: difficulty breathing becoming part of their life; listening to the voice of their own body and adapting as necessary; rebuilding their life in consideration of regrets about the past; ongoing efforts to maintain their current quality of life; feeling thankful for being able to lead a normal life; having hope for life amid awareness of their mortality; and living with the disease while fulfilling their potential. Conclusion: This research showed that the COPD patients were ready to die due to the decline in respiratory function, but they lived proactively, looking back on their past and transforming their own way of life to survive. It suggests that holistic understanding and support of patients requires looking not only at physical symptoms but also at past life experiences. |
