Title 救急・集中治療領域の看護師が終末期患者の家族支援に対して困難を感じる状況に関する文献研究

Abstract 本研究の目的は, 救急・集中治療領域の看護師が, 救命治療から終末期ケアに移行した患者の家族支援に対して困難を感じる状況を, 文献検討により明らかにすることである. データベースは医学中央雑誌を用い, キーワードは「ICU」「クリティカル」「救急」「集中治療」「終末期」「家族」「看護師」「困難感」「葛藤」「ジレンマ」を組み合わせて, 総説・原著論文 (1999-2018年) を検索した. 分析対象は, 看護師が終末期患者の家族支援に対して感じる困難な状況を読解可能な, 具体的記述を含む文献13件とし, 記述部分を抜粋して要約し, カテゴリー化した. 20サブカテゴリーから【緊急入室が多く, 治療方針の決定や看取りまでに時間的制約がある状況】【医師主導で終末期への治療方針を決定している状況】【救命できないことに対し無力感や割り切れない思いがある状況】【患者に苦痛を与える過剰な延命治療の継続に葛藤している状況】【救急・集中治療領域のケアシステムにおける終末期ケア環境を整えにくい状況】等の9カテゴリーを抽出し, カテゴリー間の関係性をもとに構造化した. 救急・集中治療領域の看護師は, 救命治療を使命とし, 時間的制約や医学的な判断が主体となる状況で, 救命治療の断念に無力感, あるいは延命治療に疑問を感じながら, 終末期患者の家族のニーズを捉えにくく, さらに終末期ケア環境, あるいは学習支援の未整備等が加わり, 家族支援に踏み出せない状況にあった.

Title PREDICAMENTS FOR NURSES SUPPORTING THE FAMILIES OF TERMINAL-STAGE PATIENTS IN EMERGENCY CARE OR INTENSIVE CARE UNITS FIELD : LITERATURE REVIEW

Abstract Emergency care and intensive care unit (ICU) nurses support the families of patients shifting from critical to terminal care. This can entail predicaments. In this study, we aimed to elucidate such predicaments through a literature search. We searched the Igaku Chuo Zasshi database in combination for original and review articles between 1999 and 2018 using the terms "ICU", "critical care", "emergency", "intensive care", "terminal phase", "family", "nurse", "sense of difficulty", "conflict", and "dilemma". The search yielded 13 articles with readable, specific narratives of situations creating a sense of difficulty for nurses, which we targeted for analysis. We extracted the narratives of nurses' predicaments and summarized it. We extracted nine categories from 20 subcategories and were structured based on the relationship between categories: "time restrictions for treatment determination and nursing provision when there are numerous emergency admissions", "a patient's family are emotionally agitated and unable to accept his or her sudden death", "terminal care directed as the treatment strategy by the doctor", "feelings of powerlessness and incomprehension when facing inability to save a life", "conflict over prolonging excessive life-sustaining treatment that causes the patient pain", and "difficulties arranging a terminal care environment within an emergency care or ICU system." Nurses working in emergency and intensive care consider life-saving treatment to be their mission. In situations dominated by time constraints and medical decisions, these nurses felt helplessness in abandoning life-saving treatment or doubt about life-sustaining treatment. While burdened by these feelings, nurses had difficulty understanding the needs of terminal patients' families and could not begin to provide family support due to additional factors such as the underdeveloped state of the end-of-life care environment and learning support.
