| Abstract | 目的: 地域包括支援センター(以下, 地域包括)看護職がとらえている認知症高齢者の家族介護にかかわる課題とそれに対する支援方法を明らかにして, 地域包括看護職の支援方法の特徴を考察する. 方法: 一県内の地域包括の看護職に対して半構成的面接調査を実施し, データを質的帰納的に分析した. 結果: 研究参加者は看護職5名(地域包括での平均勤務年数4.4年)であった. 家族介護にかかわる課題は, 家族による認知症の発見や対応の遅れ, 認知症症状と向き合うことによるストレスや受容困難, 介護の抱え込み, 虐待や不適切な介護など7つが見出された. 課題に対する支援方法は, 別居家族の立場や心情に配慮して支援する, 家族・支援者・住民協力者による支援体制をつくる, 地域資源や制度の利用を促進する, 家族や地域住民の認知症の理解や受容を促進する, など6つであった. 考察: 地域包括看護職がとらえた課題の特徴は「本人および家族の社会的孤立」と「地域住民の認知症に対する理解不足」であると考えられた. 支援方法の特徴として, 社会的に孤立し自ら支援を求めない本人・家族にアウトリーチにより支援する, 地域の協力者や資源と繋いで地域生活の基盤をつくる, 地域住民の認知症の理解を促進し本人と家族が安心して生活できる地域を目指すことが考えられた. これらはいずれも認知症高齢者と家族が社会から孤立せずに安心して生活できる地域共生社会に向かう看護実践であると考える. |

| Abstract | Purpose: To identify the challenges of family care for older patients with dementia and the support methods employed by community health nurses working at community general support centers, to examine support method characteristics. Methods: Semi-structured interviews with five nurses working at centers (mean duration = 4.4 years), in one prefecture. Results: Seven family care challenges were identified, including delayed familial recognition and responses, familial stress and difficulty accepting dementia, responsibility for all care for the patient with no support, and elder abuse or inappropriate care. Six forms of support were identified, including considering the perspectives and feelings of families living separately, creating support networks of families and others in the community, encouraging the use of community resources and social systems, and promoting acceptance of dementia among the family and community. Discussion: Family care challenges were characterized by "social isolation of patients and their families because of not seeking help themselves" and a "lack of understanding of dementia among community residents." The characteristics of support methods were "providing support through outreach for patients and their families who became isolated," "encouraging community life by connecting patients and their families with cooperators and resources in the community," and "promoting a better understanding of dementia in the community to help patients and their families live without any worries." These nursing practices promote inclusive societies in which patients and their families can live without social isolation. |
