| Abstract | 地域包括ケアシステム構築に基づき地域完結型の医療が推進されるなか, 急性期病院では短期間内での質の高い退院支援が求められている. 本研究の目的は, 急性期病院で退院支援を要する患者に対して行われている, プライマリーナース(PN)の退院支援に関わる看護援助を明らかにすることである. 分析対象データは, 入院時に退院支援が必要と判断された患者5名とその家族に対して, 筆頭著者がPNとして実施した看護援助とした. 看護援助実施に際しては, 本研究で定めた看護援助の方針に沿って実施した. 実施した看護援助は, 「行為」とその「意図」を含めて詳細に記述した. 質的帰納的分析の結果, 【対象者を生活を営む唯一無二の存在として捉え, 生活場面に内在するその人らしさに常に能動的な関心を向ける】【円滑に入院生活を送り退院の準備を進められるために, 対象者に潜在しているであろうニーズを汲み取り, 働きかける】【対象者の実像に迫るために, 多職種間での情報交換によってもたらされた多角的な視点と自らの気付きを統合する】などの10テーマが導かれた. 日々状況が変化し退院が迫るなか, PNは日々の生活場面における対話を通して患者との信頼関係を深めていた. さらに, 関わりを通して汲み取った真意や意向, ニーズを多職種と共有・補完し合うことで患者中心の退院支援実現に向けた看護援助を実施していた. |

| Abstract | Since community-based medical care is promoted as part of the development of community-based integrated care systems, acute care hospitals must provide high-quality discharge support within a short time. This study aimed to examine the discharge support provided by a primary nurse (PN) in an acute care hospital. The nursing care support provided by the PN (the first author) to five patients and their families, identified during admission as candidates who would need discharge support, was analyzed. Discharge support was performed following the nursing care provision policy defined in this study. The provision of nursing care support - both action and intention - was described in detail. Using qualitative inductive analysis, ten themes were extracted, including "considering patients as the only ones who manage their daily lives, and always actively focusing on the personhood inherent in everyday life"; "understanding and addressing patients' potential needs so that their stay in the hospital is comfortable, and they are prepared for discharge"; and "integrating one's own awareness with the multifaceted perspectives afforded by information exchange with patients and diverse professionals to better understand patients' health conditions and perceptions." As patients' status improved and discharge approached, the PN deepened the relationship of trust with patients through dialogue about their daily life situations. Furthermore, by sharing and complementing patients' true intentions and needs, drawn from relationships with different professionals, nursing care was provided to realize patient-centered discharge support. |
