| Abstract | 本研究の目的は, インスリン療法を継続し認知機能低下が疑われる高齢者に対する糖尿病外来看護師の看護援助を明らかにすることである. 研究者自身が実践した看護援助をその判断や意図とともに記録し分析するデータとし, 援助の意図を基に質的帰納的に分析した. 看護援助の対象患者10名(平均年齢80.4±4.4歳)の個別分析で抽出された229個の中カテゴリから全体分析を行った結果, 22個の大カテゴリからなる4つのテーマ「安全にインスリン自己注射を継続するための対策と評価」, 「安心して受診するための擁護者としての関わり」, 「認知機能の低下と残存する認知機能の見極め」, 「起こりうる変化の予測と支援体制の構築」が得られた. 以上の結果から, インスリン療法を継続し認知機能低下が疑われる高齢者に対し, 外来看護師は, インスリン自己注射を継続するうえでの安全性の確保に努め, 安心して受診できるような関わりを通して対象者を支えていることが示された. そして, 各対象者の残存する認知機能に応じて介入の範囲を見極めながら, 先々の変化を見据えて支援体制を構築する必要性が示唆された. |

| Abstract | This study aimed to clarify the nursing assistance provided by outpatient nurses specialized in diabetes care to older adult patients who are receiving insulin therapy and exhibit signs of cognitive decline. The researcher documented her own nursing practices, including judgments and intentions, as data for qualitative, inductive analysis. Overall analysis of 229 sub-categories extracted from individual analysis of 10 patients (mean age: 80.4 +- 4.4 years) who received nursing assistance revealed four themes comprising 22 major categories: "Measures and evaluation to continue insulin self-injection safely," "Involvement as an advocate for safe medical visits," "Identification of cognitive decline and residual cognitive function," and "Prediction of possible changes and establishment of a support system." These results indicate that outpatient nurses play a supportive role for older adult patients receiving insulin therapy and experiencing cognitive decline. They strive to ensure the patients' safety during self-injection of insulin and facilitate hospital visits with a sense of reassurance. Furthermore, the study highlights the importance of developing a support system that accommodates future changes, while considering the individual's cognitive capabilities. |
