| Abstract | 「目的」:短期間で看取りを迎えた患者と家族に対する救急医療機関の一般病棟における看護援助を明らかにし, 突然臨死期を迎えた患者と家族に対する看護援助への示唆を得ることを目的とした. 「方法」:質的記述的研究デザインで実施した. 第二次または第三次救急医療機関で5年以上の勤務経験を有する看護師を対象に半構造的面接を実施し, 質的帰納的に分析した. 「結果」:研究に参加した6名のデータから, 短期間で看取りを迎えた患者と家族に対する救急医療機関の一般病棟における看護援助として147個の初期コード, 48個の二次コード, 23個の三次コードが得られ, 13個のサブテーマ, 6個のメインテーマに統合された. メインテーマは, 【サインの察知】, 【期を逃さない】, 【きめ細かい情報共有】, 【選択の尊重】, 【外観や信仰などその人らしさの尊重】, 【最期を迎える体制づくり】であった. 「考察」:看護師は, 動揺する家族は情報に対する誤解を生みやすいことを認識して看護援助を行っていることが示された. 患者・家族に精神的負担をかけることなくニーズに合った対応をするために, 看護師間で患者・家族の情報を共有する重要性が強調された. 「結論」:救急医療機関の一般病棟の看護師は, 突然臨死期を迎えた患者と家族に対し, 看護師の関わり自体がストレスの要因となることを避けながら不安定な家族の思いに寄り添うために, きめ細かい情報共有を重視していることは新しい知見となったといえる. |

| Abstract | [Objective]: This study aimed to elucidate nursing support provided to patients who are dying within a short timeframe and their families in a general ward of an emergency hospital and to consider suggestions for enhancing nursing support. [Methods]: A qualitative approach was employed along with purposive sampling of six nurses experienced in providing end-of-life care in a general ward of an emergency hospital. Semi-structured, face-to-face interviews were conducted from August to December 2021. All interviews were transcribed verbatim for thematic analysis. [Results]: Six themes were identified: [Detection of signs], [Timely responsiveness], [Detailed information sharing], [Respect for individual choices], [Respect for individuality, such as appearance and faith], [Preparation for the final moments]. [Discussion]: The results of this study indicate that nurses provide nursing support to emotionally distressed families, recognizing their vulnerability to misinterpretation of information. The importance of sharing patient/family information between nurses was emphasised to ensure that needs are met without placing an emotional burden on the patient/family in the short time available to build trusting relationships. [Conclusion]: It is a new finding that nurses in a general ward of an emergency hospital focus on detailed information sharing in the sudden near-death period to be close to the family's unstable thoughts, while avoiding the nurse's potential to contribute to their stress. |
