Title 幼児期に先天性心疾患手術を受ける患児の主体的なレディネス発達を促進する看護援助の考案

Abstract 近年, 成人先天性心疾患患者の数は着実に増加を続け, 彼らの生涯発達に伴う課題が指摘されている. 先天性心疾患では, 慢性疾患として生涯の疾患管理を要する一方, 幼少期に集中する治療や手術, 疾患そのものの影響により, 患児の主体性を高めにくい. 本研究は, 幼児期に先天性心疾患手術を受ける患児の主体的なレディネス発達を促進する看護援助として, 学習材を用いた看護援助を考案することを目的とする. 疾患や手術に関する学習への援助により, 患児が自分なりに手術の体験を通じて心疾患に対する関心・理解を高め, 生涯に渡る療養行動に対して主体的な姿勢を築くことを目指す. 看護援助は, 周手術期の患児が示した情動反応とレディネスをもとに草案を作成し, 看護援助に参与する7領域の対象者(臨床看護師, 小児看護専門看護師, 臨床心理士, 小児循環器専門医, 心臓血管外科専門医, 成人期先天性心疾患患者, 先天性心疾患をもつ幼児の母親)への半構造化面接を経て洗練を図った. 対象者11名の評価では, 看護目標との一貫性, 幼児期の学習としての適切性, 病態学習の重要性が支持される一方, 病態学習に向けた援助の不足, レディネス発達を促進する学習内容の不足, Preparationとの類似性が指摘された. 患児の反応とレディネスより考案した看護援助は, 患児の主体的な学習サイクルに働きかけ, また複数領域の視点を統合した看護援助は, 多角的な要素を網羅した援助へと洗練されている.

Abstract In recent years, the number of adult patients with congenital heart disease (CHD) has been steadily increasing, and problems that occur with their lifetime have been noted. CHD requires lifelong management as a chronic disease; however, the symptoms and treatment of the disease in early childhood likely inhibit individuals' acquisition of independence. This study considers a nursing support method to promote independent development of readiness in preschool children undergoing CHD surgery. The goal is to enhance children's own readiness through the medical experiences related to CHD surgery, and to promote their own independent attitudes and skills regarding lifelong management of their condition. The nursing support considered in this study was initially developed based on the coping and readiness demonstrated by children undergoing CHD surgery. It was then refined following semi-structured interviews with 11 participating specialists, including registered nurses, Certified Nurse Specialists, psychologists, pediatric cardiologists, cardiac surgeons, adults with CHD, and mothers of preschool children with CHD. The nursing support considered in this study is expected to promote children's own readiness, and it has been refined with a multilateral perspective through the specialist evaluation process.
