Abstract	「抄録」本研究は, 集中治療領域において多職種連携によるせん妄対策を導入している病院の, 活動内容の実際とシステム導入過程を明らかにし, 普及に関連する要因を検討し提示することを目的とした事例集積による質的研究である. 7病院から協力が得られ, システム構築に中心的に携わった看護師と集中治療室の看護管理者に半構造化面接を行った. 活動内容は内容分析を行い, 導入過程はロジャーズのイノベーションの普及理論を用いて分析を行った. 多職種で行うせん妄対策の内容は, 9カテゴリーで形成され, プロトコルの構築や医療者への教育, 患者への直接的な介入等多岐にわたるものであった. せん妄対策システム導入過程は, 33カテゴリーで形成され, 専門的な教育を受けた医療者が組織内の課題を知覚し多職種へ連携を求めた段階から開始し, 次段階へと進んでいた. 組織全体への普及に向けて, システム導入による成果の現れ等の促進要因がある一方で, システムを受け入れない医療者の存在, 医療者の移り変わりによる不安定さ等, 普及を阻害する要因が存在し, 組織にシステムを根づかせる為には, 継続した教育活動が必要であることが示唆された.

Abstract	The purpose of this study was to clarify the actual activities and system introduction processes in hospitals that have introduced delirium countermeasures in the field of intensive care through a multidisciplinary approach and to determine the factors related to dissemination. This qualitative case study included seven participating hospitals. We interviewed nurses who were mainly involved in the system construction and nursing managers in the intensive care units. The multidisciplinary activity interventions were analyzed for content, and the process was analyzed as per the theory proposed by Everett M. Rogers on the diffusion of innovations. Multidisciplinary interventions for delirium consisted of nine categories covering a wide range of content, including protocol construction, education of medical staff, and direct intervention for patients. The process of introducing delirium countermeasures through a multidisciplinary approach comprised of 33 categories. The system started with the stage where professionally educated medical staff perceived issues within the organization and asked other occupations for cooperation before proceeding to the next stage. The introduction of the system led to improved results. Although there were factors that promoted dissemination throughout the organization, there were inhibiting factors such as certain medical staff who did not accept the system and instability due to the change in medical staff every year. It was suggested that continuous educational activities are necessary for the dissemination of this system.
