Title 関節リウマチ患者の関節炎に対する低強度筋力増強運動の効果

Abstract 「要旨」関節リウマチ (rheumatoid arthritis, RA) の関節炎に対し下肢の低強度筋力増強訓練プログラムを作成し, その効果を検証した. 対象患者 : 圧痛関節および腫脹関節が1個以上ある, 分子標的薬, 従来型合成抗リウマチ薬は, 種類用法用量が評価開始前3カ月以上変更されていない, 副腎皮質ステロイド薬と非ステロイド性抗炎症剤の評価開始前4週の用法用量が一定である. 「結果」 : 28名ですべて女性であった. Disease Activity Score in 28 joints-erythrocyte sedimentation rate (DAS28-ESR), Disease Activity Score in 28 joints-C-reactive protein (DAS28-CRP), Clinical Disease Activity Index (CDAI), Simplified Disease Activity Index (SDAI), 圧痛関節数 (68関節), 腫脹関節数 (66関節) は, すべて開始時に比べ1カ月後, 2カ月後の方が有意に改善していた. 上肢の圧痛関節数は開始時に比べ2カ月後に有意に低下していた. 握力と膝伸展力はどちらも開始時に比べ2カ月後が有意に改善していた. 歩行時間は1カ月後も2カ月後も有意に改善していた. 「結論」 : 女性RA患者に対する下肢の低強度筋力増強運動は, RAの関節炎に有効であり, 下肢のみでなく上肢の関節炎に対しても有効であることが示唆された.
