| Abstract | 「要旨」集中治療室に入院した患者の家族は, 突然の危機に動揺し混乱した状態にある. その中で, 家族は患者の代弁者となり代理意思決定という重要な役割を迫られる. 患者家族と医療者が, お互いの価値を尊重し意思決定を行うためには, コミュニケーション能力が重要である. 本研究は, 意思決定のICにおける看護師のコミュニケーションを観察し, IC中の看護師の思考を効果的なコミュニケーションアプローチとされている5Step-Approach (VALUE) の視点から分析することで, 今後のICでの患者・家族支援に活かすことを目的とした. 同席した看護師を対象に, IC中の看護師の言語的・非言語的コミュニケーションを参加観察法で実施し, IC後のインタビューで看護師の思考を5Step-Approach (VALUE) の視点で質的記述的分析法により分析した. 結果として, 参加観察によって看護師の非言語的コミュニケーションが分析され, IC後のインタビューによる看護師の思考の分析結果は【家族の思いを知る】【家族の立場で考える】など5カテゴリーが抽出された. 共同で意思決定を行うには, ICで患者家族が話すことが重要である. そのためには, ICの中で看護師が5Step-Approach (VALUE) の視点を今後の支援の中で取り入れることが有効ではないかと考える. |

| Abstract | [Abstract] The family of a patient admitted to the intensive care unit is often confused due to sudden crisis. Under the such circumstances, the family is forced to make an important decision. The purpose of this study was find the better approach of supporting decision making process for the patients' family. [Method :] Five step approach (Value) was used for analyzed linguistic and nonverbal communication and nurses' thoughts between family member and medical staff (nurse) during informed consent (IC). [Results :] There was nonverbal communication but no linguistic communication. From the analysis of nurses' thoughts, five categories of [knowing family thoughts] and [thinking from a family standpoint] were extracted. [Discussion :] In order to improve nursing intervention during IC, effective support such as 5step approach seemed to be vital. |
