Abstract	「要旨」本研究では一般病棟における終末期がん患者が最期までその人らしく生きることができるよう自立・自律を支えるチーム看護の在り方を検討することを目的とする. 病棟看護師の終末期がん看護に対する意識・看護ケアの現状を明らかにするため質問紙調査を実施し, 調査結果を看護師間で共有した. 病棟師長とリーダー看護師と共に取り組み方法を検討し, 既に行われているカンファレンスや病棟会を活用して4事例に取り組んだ. カンファレンスでは, 患者の生活背景や価値観等を共有し患者理解を深め, 病気や死と向き合い残された人生を生き抜くことを支えた. 振り返りカンファレンスでは, チーム看護を自立・自律の視点で振り返り評価した. 取り組み終了後の評価は, チーム一丸となって患者を支えることができた等であった. 終末期がん患者がその人らしく生きることをチームで支えるためには, 患者の価値観や考えをチームで共通理解した上で身体状態の変化に合わせ残された機能を生かした支援を行うこと, がんの診断期から病状の変化の度に担当した看護師が患者の病状の受け止めを把握し患者ががんと共にどのように生きていくか主体的に考えられる時間を確保し, そのために業務調整を行うこと, それらをチームで情報共有しケアを検討していくこと等が求められる.

Abstract	[Abstract] The purpose of this study was to examine the ideal way of performing team nursing that supports terminal cancer patients in living like themselves until the end of life through nursing efforts that support the independence and autonomy of such terminal cancer patients in the general ward. A questionnaire survey was conducted to clarify the awareness of ward nurses regarding terminal cancer nursing and the current state of nursing care, and the survey results were shared among these nurses. We examined the approach with the ward chief and the leader nurse, and worked on four cases by utilizing conferences and ward meetings that were already being held. At the conferences, we shared the patients' life backgrounds and values to deepen the nurses' understanding. Nurses helped patients face their illness and death. At the retrospective conference utilizing the ward meeting, team nursing was reviewed and evaluated from the perspective of independence and autonomy. The evaluation after the nursing efforts found that the team was able to support their patients. In order for the team to support the way of life of patients with terminal cancer, the team needs to have a common understanding of the patient's values and ideas, and engage in support that utilizes the patient's remaining functions as the patient's physical condition changes. Every time the condition changes from the stage of cancer diagnosis, it is necessary for the nurse to understand the patient's perception of the condition and to secure time to think independently about how the patient will live with cancer. For that purpose, daily work adjustment on the part of the nurse is necessary. Then, the patient's situation needs to be shared with the team and the patient's care needs to be considered.
