| Abstract | 「はじめに」典型的な経過をたどった, 抗NMDA受容体(NMDAR)抗体脳炎の症例を提示する. 20代女性. 高校卒業後就労され, しばらくして結婚した. 同時期に両側卵巣奇形腫が発見され手術予定であった. 肉親が亡くなりショックをうけた. 10日後, 「好きな音楽が流れている, 部屋がまぶしく見える, 歪んで見える」などの異常感覚があり, 徐々に行動のまとまりがなくなった. 数日後, 精神科病院に医療保護入院. 著しい興奮の後, 無言無動となり緊張病状態となった. 数日後, 全般性強直間代痙攣があり大学病院に転院. 呼吸抑制を来し挿管となった. 髄液より抗NMDAR抗体陽性(1:1280倍以上)であり, ステロイドパルス, 腫瘍摘出, 血漿交換, 免疫グロブリン大量療法(IVIg)を施行したが3か月間意識はなかった. その後, 転院を繰り返しながらIVIgを繰り返す中で, 発症1年3か月で意識を取り戻したが, 幼児のような情動や行動の問題があり, 精神科に入院を続けている. |
