| Abstract | 「はじめに」コンピューター断層撮影 (computed tomography [CT]) は, 多列検出型CT (multi-detector row CT [MDCT]) の普及によって任意の断面での評価が可能となった. 核磁気共鳴画像 (magnetic resonance imaging [MRI]) の撮像技術も進歩し, MR胆管膵管撮影 (MR cholangiopancreatography [MRCP]) を含めた腹部MRIの画質も向上した. これら画像診断機器の進歩により, 膵嚢胞を日常の画像診断で目にする頻度が増えてきた. 腫瘍性膵嚢胞は漿液性か粘液性かが臨床的に重要であるが, 画像診断では嚢胞内容液の性状を直接的に推定できる方法は確立されていない. 従って, 良性の漿液性嚢胞腫瘍と悪性のpotentialをもつ粘液産生性の膵嚢胞との鑑別は形態診断が主となる. そこで, 鑑別診断で陥りやすいピットフォールを把握しておくことも重要である. |

| Abstract | [Abstract] Characteristic imaging findings, diagnostic pitfalls, and differential diagnoses of cystic lesions of the pancreas were described. It is important not to mischaracterize branch duct intraductal papillary mucinous neoplasm (IPMN) as serous cystic neoplasm (SCN). In addition, care should be taken for the development of pancreatic ductal adenocarcinoma concomitant with IPMN, especially for follow-up cases. Furthermore, the radiological differential diagnosis between mucinous cystic neoplasm (MCN) and non-neoplastic pancreatic cysts such as lymphoepithelial cyst and epidermoid cyst may not be straightforward because imaging findings overlap. |
