| Abstract | 「はじめに」ストレスにあふれた現代社会において, うつ病と不安症は増加の一途をたどっている. 両疾患は, 現在世界中に不安をもたらしている新型コロナウイルス (COVID-19) の影響で, 今後いっそう増加することが憂慮されている. うつ病, 不安症治療の二つの大きな柱は薬物療法と精神療法であり, 薬物療法の主流は以前の抗不安薬から現在はSSRIやSNRIなどの新世代抗うつ薬へとシフトしつつあるが, これらの薬剤によってもなお治療反応が不十分な患者も少なくない. 一方, 精神療法のなかで有効性のエビデンスが集積しているのは認知行動療法 (Cognitive Behavior Therapy : CBT) であり, 多くの治療ガイドラインで薬物療法と同等に推奨されている. 本稿では, うつ病, 社交不安症 (Social Anxiety Disorder : SAD), そしてDSM-5などの最新の診断基準では不安症カテゴリーから独立した形をとるが, もともとは不安症の一つとして定義されていた強迫症 (Obsessive-Compulsive Disorder : OCD), これら3つの疾患に対するCBTを中心に解説したい. |

| Abstract | [Abstract] Recently, the number of patients with depression and anxiety disorders is steadily increasing under the stressful modern society. Further increase of these disorders are worried due to the COVID-19. Though the first lines of pharmacotherapy for these disorders are new generation antidepressants including SSRI and SNRI, not a few patient response inadequately. On the other hand, regarding psychotherapy, there are numerous evidences of the effectiveness of cognitive behavior therapy (CBT). In most treatment guidelines, CBT are recommended as a first line treatment, as well as pharmacotherapy with SSRI/SNRIs. In this review, I will provide comprehensive commentaries of treatment strategies including CBT on depression, social anxiety disorder and obsessive-compulsive disorder. |
